2 input headphone amp

Does anyone know of a 2 input headphone amp to accommodate my CD and phono stage with easy switching between the 2?
I'm thinking of this, rather than using th QED input expander suggested in another thread.

It would be very simple to make a small external switch.... depends how important minimising number of boxes and connnections is to you
